How to File a Claim for Damages

Claim for Damages (Tort) Claim

Individuals who believe they have been harmed or have suffered a loss as a result of negligent actions by the City of Chehalis can submit a claim for damages (tort) claim. The City Clerk is the appointed agent for the City  to receive such claims.    

The City's insurance provider, Washington Cities Insurance Authority, objectively determines the City's liability for claimed injuries.  It fairly compensates claimants for damages when liability is supported by evidence and denies claims when liability is unsupported.  

DO NOT USE THIS FORM if you were harmed by another entity (for sample: the United States Government, Washington State, another country, another state, another county, another city, or private citizen)  You must file your claim against the other entity or entities.  

Documents will not be returned, please keep copies of everything you submit.

Please note that the Claim for Damages form is subject to public disclosure pursuant to RCW 42.56.

How to file a claim with the City of Chehalis

The City of Chehalis Claim for Damages may be filed online, by fax, by mail, or in person.

  • Submit your completed Claim for Damages and supporting documents online to: kmackie@ci.chehalis.wa.us., or   
  • Submit your competed Claim for Damages and supporting documents by Fax at the following fax number: 360-748-0651, or
  • Submit your completed Claim for Damages and supporting documents by mail to: Attn: City Clerk, City of Chehalis 350 N. Market Blvd., Chehalis, WA  98532
  • Submit your completed Claim for Damages and supporting documents in person between 8 a.m. and 5 p.m. Monday through Friday at the following address:  350 N. Market Blvd., Chehalis, WA  98532

 The City of Chehalis Claim for Damages must be signed by one of the following:

  • The Claimant 
  • A person holding a written power of attorney from the Claimant 
  • The attorney in fact for the claimant 
  • An attorney admitted to practice in Washington state on the Claimant's behalf, or
  • A court-approved guardian ad litem on behalf of the Claimant
